Tutorial Feedback - Github issues and feedback survey

Currently we have the "Share your feedback" link at the footer of the resources page of the tutorial so users can leave their feedback as an issue in this github repo. However, when we get the feedback survey published (#423) we will have two distinct places in the page to ask for feedback from the user: the feedback survey form and the footer.

How can we ask feedback from the user without giving so many alternatives to the user when they finish a tutorial?
