feat(cohorts): add activity logs
Problem
A user has reported cohorts changing in unexpected ways.
Changes
Adds activity logs to changes to cohorts from the UI, so we could rule out accidental mistakes.

I built this a while ago but never finished. It should help with a customer case, at least to rule out some cases. Ready for review now.

It would, but then I'll keep this PR pending for another 2 weeks đ . Let's get it in and add when someone requests it.
