Update Gravity Forms UX

Currently our settings in the Gravity Forms editor are no longer recommended, but they also are not rendering inside the new metabox style containers in latest GForms.

Update them to match and also add deprecated notices, disable the fields to prevent usage going forward but still show any previously set values. If enabled, make those fields still editable.
