There's been issues with the QCH build for some time: p12tic#47, p12tic#79, p12tic#129 I don't use the QCH file personally, so I'll most likely not put much work into that....
Sorry about not responding to your PR, I haven't been very actively working on this. I'll take a look whenever I get to working on issues.
It would also be nice if the whole file was formatted when the current selection is empty. edit: I just realized there's already a pull request for that.
We could also check relative links to make sure they point to existing files.

That sounds like a great idea. There's already some index information available for function names, so adding search for that shouldn't be impossible. However, I don't know about full text...
I would like to second this, but instead of notes I would use it for other kinds on non-password secrets like SSH keys or tokens (even config files containing tokens).
