Replace or Remove old isDefinedBy placeholders

Lots of terms originally in the GitHub JSON-LD have this:

<http://opaquenamespace.org/ns/subject/StevensonDonaldMcCornack> <http://www.w3.org/2000/01/rdf-schema#isDefinedBy> <http://opaquenamespace.org/VOCAB_PLACEHOLDER.nt> .

which isn't exactly useful.

there are probably a lot less of these but looks like seeAlso also has placeholder object http://opaquenamespace.org/ns/creator/ButchkoBrandon i.e. see also seeAlso
