Teams creation fails - with error message "Team already exist"

I have a tenant with this setup. No errors in script, no errors in flow or power app. A lot of request has been pushed throug and no error. But a couple of requests ends up as failed in the teams request list. And the error message says Team creation failed - "Team already exist" But it doesnt.. Not anywhere.. So why does this happend? It passes the Check Team Availability..

Please help!

@Humpeklumpen So these seem to be two issues, the first one is an issue with the logic app in Azure. The second is related to adaptive cards.

For the first, please check the ProcessTeamRequest Logic App in the azure portal, check the run history for any failed runs. This should tell you what has failed. Feel free to post that back here.

For the second error, have you made any changes to this flow at all?
