Add ability to add a note when giving freeleech tokens to someone

Right now, when giving freeleech tokens to someone else, you may only input the name, and it sends the tokens with a generic message. It would be beneficial to add an ability to add in a message so that the receiver would know why they get the tokens (I've received a couple of confused PMs as to why I sent people tokens in the past week when testing something).

The interface should be similar to the custom title purchase where when you click the button, it takes you to a new screen where it shows an input for the username (required) and a textarea for a custom message (optional).
