Update Observation table, ETL conventions for a person's "family history" event

Per this Themis issue, the following guidance has been given on how to properly bring family history into the Observation table. The observation_concept_id should be ‘4210989', Family history with explicit context and value_as_concept_id should be the related procedure, condition etc.
