Country Vocabulary guidance needs updating

Currently, the country_concept_id guidance for CDM v5.4 points us to standard concept_ids WHERE domain_id = 'Geography'. However, it should point us to WHERE standard_concept = 'S' AND vocabulary_id = 'OSM' AND concept_level = '2nd level'. It's the 2nd level of the OSM vocabulary which contain the country data. Other levels of the OSM vocabulary might match on concept_name, but don't have the same meaning (town or county instead of a country).

So, there is talk about adding SNOMED concepts to the Geography domain. If that happens, then we should give the above guidance AND say there are also acceptable concepts with domain_id = 'Geography' and vocabulary_id = 'SNOMED'.
