Healthcare process: what is meant by "directly involves"?

Does a pharmacist dispensing a prescription "dircectly involve" the treatment, diagnosis, or prevention of disease or injury of a patient? That is, is dispensing a prescription a healthcare process?

What about Good Samaritans? Does their help of a an injured person automatically make that person a patient? Or is that not a healthcare process?

What about a patient taking medication that was prescribed? Direct involvement?
