handle pthreads-win32 download/fix/build during cmake

w/r/t the "add #define HAVE_STRUCT_TIMESPEC" instruction for win32 builds: it's a dodgy way to work around an upstream bug

i suspect https://cmake.org/cmake/help/latest/module/ExternalProject.html may be a good feature to use for this
