BUG: 'Find your Home Care Taker' form not optimized for dark mode.

What went down?

The 'Find your Home Care Taker' form on the website appears to have visual inconsistencies when viewed in dark mode. Specifically, the input fields for 'SELECT MEDICAL NEED', 'SELECT YOUR CITY', 'CHOOSE DATE', and 'CHOOSE TIME' are rendered with a white background, which contrasts sharply with the dark theme of the rest of the website, hampering the user experience.
