Add toggle to collect reporting user information on bug report

If the user chooses to be anonymous, then remove the email text field. Else, automatically collect its faculty and institutional email and Redux Store/Provider contents (inject in the Sentry description, not GH!) and allow them to include an extra email through the already existent email text field.
