Use BsonDocuments in the iterator

If we get info back out of the driver as a MongoCursor<BsonDocument> we can hand each document off to the codecs with out our own DecoderContext instances with caching information to allow for proper caching/fetching of duplicate/cyclical references. This will still require changes to the driver so it's all very speculative.
