Adding cover image to text generates a text added tile

When adding a cover image to a text record, the system creates for that text a new text-added tile that appears as recent activity. Adding/adjusting record should not re-create the tile.

In this particular case, to re-create this, load a text into a project, delete it's text-added tile, add a cover image to the text. The text-added tile is made anew in activity.
