ref: add new fields to data access request form to support bridge2ai data use request process

For the bridge2ai project we need some extra fields for review of the data use agreement form by the DAC.

Several of the required fields are in the credentialing form, so I am not adding it here. Also noted that the credentialing form aspects for data use details, which seems like it should be associated with a specific dataset rather than credentialing.

Happy to make any changes as needed, but thought I would get this process started.

from a spec perspective, we would want:

• a credentialed user needs to submit this form
• the user should have an affiliation
• the review should include user details (including afiiliation), credentialing details, and the information collected in this form
