Improve organisation of the authorship page (make personal affiliation form easier to find)

Currently the authorship page at http://localhost:8000/projects/oQDnAyjnJOtM3uPSYkb2/authors/ combines forms for (1) inviting co-authors and (2) adding information such as corresponding author and personal affiliations.

The affiliation form in particular is fairly well hidden (at the bottom of the page). I was just completing a test submission, and it took me a little while to find where to add affiliation after being prompted on the final project preview.

We should think about how better to organise this page, with a focus on making the personal affiliation form easier to find.
