Add rxnorm concept mapping

This pull request adds a derived mapping table for medications. The CSV file contains itemid to omop_concept_id and omop_concept_code for rxnorm mappings codes. I manually mapped the RxNorm codes using RxNav and Athena. Mapping codes were reviewed and verified by @xborrat.
