Finish incomplete documentation for apacheApsVar

In the documentation for apacheApsVar table, the text under the "urine" section contains incomplete sentences:

"Each of the following variables are assessed a score based on its variation from a predetermined set or mid point. Variables that occur within the first APACHE day are included. If there are no laboratory variables in the first APACHE day then variables from"

And there seems to be no documentation for the "meds" section.

I'll put in a pull request for this. In the short paragraph, I would just remove the last sentence.

The meds variable is a variable signifying that GCS wasn't measured because the patient was sedated.
