[Bug] The date field accepts unrealistic year inputs in OPD registration form.

Bug Description?

1. The 'date' field in OPD registration is currently accepting large & unrealistic year inputs (as can be seen in screenshot).
2. If its date of appointment then 'date' should be renamed as 'Appointment date' for more clarity.
