[Feat] Add CAPTCHA to OPD registration form

Feature Description

Currently, the form is susceptible to automated bots that can submit fake/ multiple registrations, so to prevent this, there is a need to implement CAPTCHA.

Use Case

1. It will ensure real individuals are registering for the appointments.
2. It will maintain data integrity & will prevent spam entries.
