Form widgets aren't showing up

I'm not seeing the input fields, forms etc in the UI - looks like it's in your source code...

I decided to disabled the form components until they are properly tested.

If someone wants to ensure that they work as expected and possibly fix any bugs in a PR I will review and merge asap.
