Add examples with gpu global indexing for both kernel and launch

In a new example it may be useful to add a global indexing example to point users to. There are some for launch but a focused example may be useful for folks browsing through examples. The thought came from this interaction: https://github.com/LLNL/RAJA/issues/1679
