Feedback form

• [ ] Add a form to send an email to us
  • Have a form element for optional reply-to; autofill with account email address if user is logged in
  • Link to this form wherever we ask for feedback
  • Include three modes of soliciting feedback everywhere we do so:
    • “Send us a message” - link to web form
    • “File an issue on GitHub” - link to new issue on GitHub
    • “Email us” - link to same email that first option uses implicitly
• [ ] Ask IT to create a [email protected] email address
• [ ] Add a navbar link to contact us (megaphone? “Bug icon”?)
