Create README for Transitioner\Trans\Trans project

The README should contain the following elements

• The title of the project
• A brief decription of what the project shows
• A visual of the running project. This may be either a static image or a gif (ScreenToGif may be helpful)
• When possible a brief explanation of how the code works.
