Experimental: add option to always use 15 channels

Use case: start with a chord of 1 note and then bend to a chord of 15 notes without any additional MIDI note on events.

This is experimental because I have not investigated how the MIDI protocol handles starting multiple notes (on different channels) at the same pitch.
