Description

In order to be able to be registered as a Protector, the nominated protector needs to be verify their identity. We need to create a form that includes all the required details for the user to do so.

Acceptance Criteria

an option to see a heading: Verification process
an option to see tabs of 4 steps: About verification, Personal data, Documents, References
an option to see an active state for current step
an option to see a step number and step title
an option to see a card with email
an option to see a divider lines
an option to see and click Continue/Back button
- Continue button should take user to next step
- Back button should take user to previous step
- Back button is not visible on Step 1

Step 1:

an option to see a description of Step 1
an option to an information about data

Step 2:

an option to see users Colony username and avatar
an option to fill in form fields: name, address (max 90 chars, placeholder text), telephone, email
an option to select Yes/No answer
- only one answer can be selected

Step 3:

an option to see an address used in previous step
an option to upload a file confirming the stated address (acceptanble format: png, pdf, jpg, up to 9MB)
an option to fill in form fields: passport number
an option pick a country using a dropdown menu
an option to upload a file with passprot scan (acceptanble format: png, pdf, jpg, up to 9MB)
an option to pick a country of tax residency using a dropdown menu
an option to fill in form fields: Tax ID
