Added application form

Description

This PR adds Incorporation Page where on the left side user can see the application form. The form consists of the following fields:

• input for the name of the corporation,
• two inputs for the alternative names,
• textarea to describe the purpose of the DAO (max 90 chars),
• select to nominate protector (with 'plus' icon, user can add max 5).

These fields are visible all the time. There are also 2 more fields visible only when more than one protector is selected. And they are:

• select to choose main contact (can only be selected from nominated protectors),
• radio buttons to choose how documents should be signed.

Form validation (and "Submit" button) will be added in different PR. Initial and ongoing cost are hardcoded.

To go to the Incorporation page you have to type in the address bar "/incorporation/create" after colony name. e.g. "..../colony/{colonyName}/incorporation/create"

PR with redirect to this page is not merged yet, so for now it is only available in this way.

SingleUserPicker added here is just a wrapper around UserPickerWithSearch and trash icon. It's easier to add logic to remove items in a separete component.
