Incorporation: As a user I can use the Sidebar Form, to fill in all the needed information for application

Description

In order to be able to start the application process, a number of details are required, these include the name of the required incorporated company as well as some alternative names as a backup which speeds up the process of registration. There also needs to be a protector nominated who will be in charge of ratifying the decisions of the DAO. If there are multiple protectors, then there also needs to be a main contact nominated for the application.

Acceptance Criteria

an option to see incorporation cost details (initial/ongoing)
an option to fill in the fields in the form
- Name of the corporation
- Alternative names
- Describe the purpose of the DAO (max 90 chars)
an option to use dropdown menu and nominate protector
an option to add more than one protector (max 5)
an option to nominate main contact, if more than one protector is nominated (prepopulated with first nominated protector)
- main contact can only be selected from nominated protectors
an option to delete nominated protector and *main contact *
an option to see an (i) icon and tooltip when mouseover on it
an option to choose "how documents should be signed" (this is visible when more than one protector is nominated)
- first option prechosen
- only one option can be picked
an option to use scroll if not all information is visible
