Incorporation: As a user I can see a banner with buttons to Verify or Remove protector

Description

A user who has been nominated as a Protector should see a clear notice to let them know they need to take the action of verifying their identity.

Acceptance Criteria

• an option to see a banner with:
  • users name
  • text
  • Verify button that should lead user to Verification form
  • Remove button that should lead user to Modal for removing nomination
