File issues at the new Repo!

This project has moved!

It's now being maintained by proactive volunteers from the Elixir community over at elixir-lsp/vscode-elixir-ls. All issues should be filed at that repo instead.

(Jake please leave this issue open so users can see this message without having to hit the "new issue" button)
