Titan two output bugs out and then get error E4

Hey whenever I try to use output I cant get it to work. The LCD bugs out and the light will turn off and on. When I put it in prog my pc reads everything fine including the controller. When I switch to output it goes all crazy and PC wont even recognize it. Because of this its useless. Ive tried lot of different cords have had it for 2 years and never been able to use it.
