SQL Server Counter Issues

I see that there has not been much activity here lately. Does this solution work with SQL Server 2012? I have tried this with the various counters built in. They all work with the exception of SQL Server. When I un-comment the SQL Server counters, it stops gathering all stats. I see the service start up in the log, but then nothing else. Ideas?

If your SQL instance is a named one (not default), the base name of counters changes ("MSSQL$INSTANCENAME:...")
