What happened?

There are 3 MO Disks in game. One of them the log log says its was placed on room 501 (that is the lab corridor that has the elevator to final battle), but it was a Flame Rounds that was placed on Battery's place. This room only have 1 item, the Baterry and nothing else, but if you go further on log, is says the clip in this room (that doesn't exists) was replaced to Flame Rounds
