_Page break_ button to be removed

A Page break button is part of the action bar, but it has no effect whatsoever. I propose we remove it and the related code in Pogues in order to be lean.

Pogues Model might need to also be updated.

More validation seems needed beyond the action bar, so a more complete validation is needed.

I took a more complete look at the Page break version of the app. Most of the features work as intended, but i came across two minor bugs:

• when switching from Filters to Gotos, the filter ui component (a blue bordered rectangle) didn't disappear. A reload of the questionnaire make it disappear.
• when clicking on the Merge questionnaire button, nothing happens (or more accurately it seems the modal appears / disappears at the speed of light). The second and following click work as intended.
