Add Fastify VPS Hosting + Wrapper

With the move to CF Workers by default, we lost the ability to run ikit on a VPS through fastify. This should be a setup option that automatically adds a conversion middlelayer to the application (and should have an explainer in the documentation on how to convert back and forth)
