Review "Needs Review" rules issue holder

Issues in this epic are for rules that have a Needs Review portion of the rule that triggers. The intent of these issues is to determine if we can tighten the scope on these scenarios so that they trigger less often.

@marcjohlic @tombrunet Is the goal to trigger less often or to split the rule so that it has higher confidence that it is a fail and the remaining scenarios it is a "less often" triggered Needs Review?

@philljenkins Find scenarios that are Pass or N/A and encode those into the rules so that the Needs Reviews trigger less often. If we find definite Fail scenarios in the process, we can add those violations as well.
