Fix broken headings in Markdown files

GitHub changed the way Markdown headings are parsed, so this change fixes it.

@bryant1410 thanks for these changes, I see you made an additional PR which I accepted - this one however has an out of sync branch with 1000s of merge conflicts. Looks like on your subsequent PR you were able to rebase and I had no merge conflicts.
