Mobiel App - Disaster Check in page

From the profile page when the user clicks on check into a disaster they will be brought to this screen. This screen is essential is a form that serves the same purpose as the "Make a New Commitment" form found here https://crisischeckin-d.azurewebsites.net/ (login with user: testUser pass: test)

Back End: This form will write information to the volunteer checkin DB as well as flag a volunteer as checked in to the specific disaster.
