Duplicate entries in the tree for the same item due to uppercase letters in the file name

Everything works fine, except when a file (README.adoc in my case) has uppercase characters.

• I get an item for both the true file name and a lowercased version
• When I remove the ToDo, the true file name version lingers until I refresh the tree, but the lowercase version disappears when edit completes

This seems to be true of any number of letters being uppercase in a filename.

This is using ToDo Tree v0.0.226.
