Public form: Create "high level" public form documentation

Background

To support onboarding and help the development team become effective contributors, we need to document the pattern at a high level. This documentation should clarify how the public form is structured, how its parts interact.

Acceptance Criteria:

• A high-level system diagram that visually maps out the key parts of the public form pattern.
