Public form pattern: Document uploader support

Background

Currently document upload is supported but only as discrete tasks within a step (see "supporting documents" step below). We would like to support the inclusion document uploading within a step.

Acceptance Criteria:

• Document uploader component can be included with a series of questions with one task Figma design.
• The uploaded document is included in "review" page as indicated in the Figma design
• The document uploader supports 1 or more uploaded files
• File meta data added by the user is supported
