Consider adding a direct link for requesting an origin trial creation

Currently, feature owners must navigate to their feature's detail page and select "Request Origin Trial Creation" on their origin trial stage to be navigated to a form to fill out for the origin trial request. The URL for the origin trial creation form contains the feature's ID, as well as the origin trial stage's ID. This means that we cannot link directly to the trial creation form in documentation for the feature owner to navigate to.

It might be useful to have a more generic page in the UI that prompts the user about submitting an origin trial creation request, and gives the user the ability to select which feature they would like to use for origin trial creation.
