Ask a few extra questions for aid researcher

I’m no researcher, but I can imagine researchers would want to know these extra data points (should be totally optional to fill them in in the Settings page):

• Country
• Pump or pens
• Type of insulin
• How frequently do you visit a diabetic specialist
• Number of people in the household
• Other?

We already have plans for this we just have not created an issue to track it so I will keep this open till the text of the issue we are putting together can be published.
