Tyepform/system for flagging projects

As per the stewards call, we should create some system where users can report suspicous/malicious projects. Some thoughts:

• Maybe we have this autopopulates a discord channel w/ info so we can see reports easily as they come in without needing to check somewhere new
• The form should have at least these questions: project link, why you are reporting this project (w/ an ability for the user to share links), contact info of the reporter

I'm also not sure where this form should go to be accessed. maybe we can add it to the dapp where we have the "report and issue" section, and perhaps in the ToC?
