The popup that appears on the bottom says "To protect your privacy, access is not possible". Also, as you noticed, the app never asks for access to files, only for the mic
Please try selecting your Recordings/test folder directly in this app picker dialog before you tap OK (i.e. before launching the system picker).
Sadly same result

That is very strange. The system picker shouldn't be hiding your folders on that screen.

Android 16 GrapheneOS, in app permissions, no file permissions availabke
@PanderMusubi This app doesn't use any global file permissions. This is why the startup dialog to select a folder is necessary.
But it only allows directories which I don't want to use. I want to write to subdirectory in Documents or DCIM e.g.
Having the same issue on a Pixel 8 Pro running GrapheneOS. The app initially prompts me to set the storage location for recordings, but only allows for creating folders inside Internal/Android/media. I don't want to save recordings to that location. I want to use standard folders more easily available to other apps, such as the Downloads or Music folders.
Frankly this is a dealbreaker for me. I keep installing this app because there aren't a lot of FOSS simple voice recorder apps, but every time I open it and remember that it won't let me choose a sane location to save recordings, I uninstall it again.

This is fixed for me since 1.5, sorry for the late reply.
When selecting an folder, he asks now for permission and it works as expected
It was probably a system file picker issue that got fixed by Google Play system updates. Or less likely but, it might have been fixed by https://github.com/FossifyOrg/Voice-Recorder/pull/187.
You're right. Downgrading to 1.4 and it's still fixed
