Add link to FAH user name checker

The Team: has a link to pick a team, so the Fold As: should have a link to the Name Checker so they can pick a valid FAH user name, or check for Duplicates, etc. http://folding.stanford.edu/home/fah-user-name/

Alternately, you could had a link to the name checker under the Popup window for Passkey.
