Hung at booting after CoreDisplay patch

My MacBook Pro is A1398 15 inch Early 2013, it has NVIDIA GeForce GT 650M 1024MB and Intel HD Graphics 4000 1536MB. OSX is 10.12.6 Sierra, I bought a miniDP to HDMI 2.0 active cable from amazon de which is Club 3D 1180, and my monitor is LG 4k 27UK650.

Before use CoreDisplay patch, I can get a 4k 35Hz with SwitchResX with less than 300MHz Pixel clock. Only CoreDisplay patch run successfully, Nvidia patch miss OSX 10.12.16 version. After reboot the system hung at apple logo loading. I have to boot with safe mode to restore the CoreDisplay file.

Is there any update step I missed?
