404 File Not Found After Clicking on a Submit in HTML Screen

What would you like or expect to see?

Hi, I have the following HTML screen form:

What's your name?
How old are you?
I agree

Continue

It should go to the next Screen when I click on Continue button.

What is currently happening?

It shows a 404 File Not Found Page with this message: The requested URL /api/labjs_preview/index.html was not found on this server. located at: https://labjs.felixhenninger.com/api/labjs_preview/index.html?name=s&age=18&ok=on
