Rewrite README.md to improve clarity

Wanted to improve grammar, fix old links, etc, but I also added new information (such as a quick example, which is useful for people to get an idea of the module) and restructured to make it more clear and provide some logical sections. This is more of an opinionated rewrite than the wiki fixes, so feel free to make changes as needed.
